Utah measles hits 696 as Oregon school coverage slides (June 24, 2026)

Utah's at 696. Oregon's student vaccination rate just hit a new low. Those two facts landed in the same week for a reason. If you're just joining us: Utah's been tracking a 2025-2026 measles outbreak through state, local, and wastewater dashboards, with regular DHHS case counts. Before today, the dashboard stood at 687 confirmed Utah residents across the outbreak — thirteen reported to public health in the prior three weeks. The question was whether that recent-case window would keep shrinking, or tick back up. Measles Outbreak Daily. Today — the national count is past 2,000, the elimination review that should've happened in April, and the one number out of Utah that complicates the story. Stick around for that one. When measles is detected in wastewater it means one or more people in or traveling through that area has been infected with measles and is shedding the virus. Wastewater test results can tell us when the measles virus is in the wastewater, but the results don’t tell us how many people in the area are shedding the virus. It could be one person, or it could be multiple people.

Utah's dashboard updated yesterday: 696 total now, up from 687. And the three-week trailing count is up to 18. Eighteen in three weeks. Fifth straight cycle where it hasn't flattened. At some point, a state health department has to stop treating that as a trailing count and call it a floor. But here's the parent problem, Cass — 18 cases in three weeks against a 696 cumulative total reads calm. If I'm deciding tomorrow morning whether my kid goes to the birthday party, which number am I supposed to act on? And 499 of those 696 are in 2026 alone. So whatever 'slowing down' is supposed to feel like, this calendar year is doing most of the damage. Right. Two hundred in 2025, nearly five hundred since January. The year split is where you see the acceleration. When a health department drops a number like '124 new measles cases,' does that mean 124 people got sick yesterday, or are we sometimes just watching a backlog clear all at once? Great question, because that 124 number is a real example. In January, South Carolina reported 124 new measles cases in the last three days on a single Friday update, pushing the state's total to 558, per WUNC's coverage of the outbreak. So that's the mechanic: three days of cases, one dashboard update. CDC clinical guidance says laboratory confirmation is required for all outbreak cases. And UK Health Security Agency research on the 2023-24 England outbreak — the closest published model we have — found that lags from symptom onset to test results made confirmed-case data 'inherently retrospective rather than real-time.' In practice, a big single-day jump is usually a mix: new infections, cases from earlier in the week that just got confirmed, and sometimes a backlog clearing. The honest read is: a sudden spike tells you the outbreak is active and growing. It doesn't prove transmission accelerated specifically on the day the number posted. So if the numbers are already running behind, does that mean the real-time caseload is actually higher than whatever the dashboard shows on any given day? That's the working assumption epidemiologists use: confirmed totals are usually the floor, especially mid-outbreak. What listeners should watch is the trend line over multiple reporting cycles, not any single day's number. And watch whether health departments are flagging reporting lags explicitly, because when they don't update for an unusual stretch, the silence itself is worth noting. Oregon Health Authority writes:

For the 2025-2026 school year, Oregon's rate of nonmedical exemptions for required school vaccines for kindergarteners has climbed to a record high of 10.9%—up from 6.9% in the 2021-2022 school year. Only 85.6% of kindergarteners were up to date on required vaccines, according to data, continuing an overall downward trend that began a decade ago. The steepest decline happened during the last four years—a 3-percentage point drop since the 2021-2022 school year, when the rate was 88.4%.

Oregon's nonmedical exemption rate for kindergartners just hit 10.9 percent. It was 6.9 in 2021. And up-to-date coverage is down to 85.6 percent. So when state officials say more than a third of Oregon schools are at risk of an outbreak, they're describing the policy choice already sitting in the coverage numbers. And Oregon is blue. We keep telling ourselves the exemption problem is a rural, red-state story, and here's Portland sitting at almost eleven percent. This is the cluster nobody's looking at. Five percent is the threshold where you lose herd protection for measles. Oregon's 10.9 is more than double that — across the whole kindergarten cohort, statewide. OHA's move here is to point parents at a per-school dashboard. Which I actually like — it answers the question I'd be typing at midnight: what's my kid's school sitting at, not the state average. The statewide number hides the worst schools. A 10.9 percent average means there are buildings well north of that. The dashboard is the more honest version of this story. Measles infections in the U.S. this year are on pace to easily eclipse 2025, when the country reported the most annual cases in over three decades. U.S. News is tracking the outbreak, which includes 2,030 confirmed cases and 30 new outbreaks.

U.S. News has the live number at 2,030, dated June 5 — and the headline says that puts elimination status 'at risk.' At risk. Thirty new outbreaks, forty jurisdictions reporting, and we're still narrating this in the conditional tense. And the review was supposed to happen in April. It's late June. We're past 2,000, we've got 30 outbreaks, and there's no announced decision. A bill stalling in committee, I understand — what's the holdup on a status review? That gap is the story, Matt. The U.S. has held elimination status since 2000 — twenty-six years. Losing it should hit like a sustained hurricane season. Instead, it's landing as a footnote with a hedge attached. Every school administrator reading 'at risk' thinks there's still room to wait it out. And 2,030 already beats all of 2025 — which was itself the worst year in three decades. So 'on pace to eclipse' undersells it. We've eclipsed it. The hedge is in the pacing language now. Here's Washington State Department of Health:

Measles is a vaccine-preventable disease that can spread very easily between people who are not immune to the disease. Cases of measles are rising across the U.S. and in many countries around the world. We are tracking measles cases and outbreaks in Washington state. All data are preliminary and subject to change.

Washington's posting 45 confirmed cases — but look at the timestamp. Last update May 22nd. It's June 24th. That dashboard's been frozen for over a month. And it says right on the page — updated Mondays, Wednesdays, and Fridays 'when new information becomes available.' A month of silence means either nothing happened or nobody's looking. Snohomish is at 14, Clark at 8, Kittitas and Walla Walla both at seven. That's a real geographic spread — east side, west side. And then the page just stops. Three hospitalized, zero deaths, as of five weeks ago. Set that next to Utah's 696 and a national count past 2,000, and a stale 45 leaves a blind spot between two states where the numbers are still moving. For parents, that's the problem. If I'm in Snohomish County deciding whether to send my kid to a birthday party Saturday, a number from May 22nd answers exactly nothing about this weekend.
